When you help changing someone life ( true story with my patient )
In a routine round I make every duty , I was reviewing file of a 28 years old male toke unprescribed Olanzabin ( antipsychotic ) from his friend to make him sleep after while he felt abdominal pain and some abnormal movement , he admitted as drug induce convulsion . Till now nothing suspicious to me but when his brother said there is no convulsion and it is just behavioral change and he being very aggressive after he toke that medication because he fought with all his family .
The patient was quite and have a calm personality , I started to dig deep in the medical history , he have recurrent same complain over and over behavioral change with sever abdominal pain and tea like urine , he thought he have dark spirit in his body and tried many herbal and spiritual therapy , his family thought he have psychiatric condition and start to treat him that way specially after he got fired from his job .
Basically we have young smart man preoccupied by the idea that either he have dark spirit or psychiatric condition and he start to give up in life .
I suspected a medical diagnosis and discussed his condition with consultant and in Medical morning meeting most agreed about my opinion and we order some test came positive he have Acute intermittent porphyria , it is a rare metabolic syndrome presented usually with episodic attack of anxiety, agitation, hallucinations, delirium, or depression and sever abdominal pain along with dark urine and constipation and other , it has aggravating factor and symptomatic treatment .
I went to the patient as soon as the result came and told him that he have metabolic medical syndrome witch cause all his symptom , he asked some question then start crying , according to him his life changed now , after some chat with him he started thinking about working again and in the next day talked about some engagement plan with a girl .
This man life changed because simply careful listing lead to right diagnosis while other missed it , I felt great that day ( sooo good feeling ) , I mean imagine moment like this when you feel that you truly helped someone and his life is better now worth all hard work and studying in medical school and make satisfied about what you are doing and that give you energy to do more :)
