Fixing the Batch Follow/Mute Tool due to Condenser Logic Change

Due to the logics change merged here: https://github.com/steemit/hivemind/pull/307
The previouis logics slightly changed from :

to

["", ""]
["blog", ""]
["blog", "ignore"]
["","ignore"]

so, i have updated the tool at https://steemyy.com/follow-or-mute/ so that it can push the right ops to the blockchain
